A wellness evaluation will be sent to you via the internet.  This evaluation will consist of a number of questions and assessment tools covering physical and emotional areas for you to complete and return to the wellness specialist for scoring.  Once scored the specialist will send you a treatment plan that offers suggestions regarding your wellness needs. Suggestions are made as to which available wellness services will provide you with the greatest benefit.  These services may include; but are not limited to; nutritionists, massage therapists, personal trainers, dentists, and/or physicians.
